whoRU is installed and running in the menu bar: look for the person-with-a-question-mark symbol.

Its setup assistant is open. Grant the Accessibility permission there, then press Try it now to see the panel next to a real permission dialog.

From a terminal, whoru scan /Applications/Safari.app runs the same checks without the panel.
